According to The New York Times, Ozzy Osbourne’s death certificate lists “out of hospital cardiac arrest” and “acute myocardial infarction” as the primary causes of death. The Black Sabbath frontman, 76, passed away on Tuesday, July 22, surrounded by family and loved ones.

Additional contributing factors included coronary artery disease and Parkinson’s disease with autonomic dysfunction—a condition Osbourne had been battling in recent years. The certificate was officially submitted by his daughter Aimee Osbourne at a registry office in London.

Osbourne’s funeral procession took place on Wednesday, July 30, in Birmingham, the city where his musical journey began. Family members paid tribute to the rock icon with emotional gestures, marking the end of an era for heavy metal music.

The Osbourne family confirmed his passing in an emotional statement to PEOPLE:

“It is with more sadness than mere words can convey that we have to report that our beloved Ozzy Osbourne has passed away this morning. He was with his family and surrounded by love. We ask everyone to respect our family’s privacy at this time.”

Best known for hits like “Crazy Train” and “Paranoid”, Osbourne helped shape the landscape of hard rock and heavy metal across five decades. His farewell concert in Birmingham on Saturday, July 5, marked his final performance with Black Sabbath in over 20 years—a powerful and emotional sendoff for devoted fans.
